State Police investigate a fatal car crash in the Town of Wawayanda

State Police in Middletown are investigating a fatal car crash that occurred at 5:42 a.m. on July 5, 2017 on State Route 284 at the intersection of County Route 93 and State Route 284 in the Town of Wawayanda, Orange County.

A preliminary investigation revealed a 2009 Toyota RAV4 traveling north on County Route 93 failed to yield the right of way and struck a 2005 Peterbilt 375 dump truck traveling west on State Route 284.

The operator of the Toyota, Andrew Van Dyke, 17, of Highland Lake, New Jersey, was ejected from the vehicle and pronounced dead at the scene.

The operator of the dump truck, Jeremey Hermida, 28 was taken to Orange Regional Medical Center for evaluation.

The investigation is ongoing.
