On June 23, 2023, at approximately 6:08 p.m. State Troopers responded to Route 28 and Bridge Street in the town of Shandaken for a report of a two-car vehicle crash with entrapment.

Investigation revealed that a 2002 Subaru Outback, operated by Anna C. Maguire, age 79 from Woodstock, New York was traveling eastbound on State Route 28 in the area of Bridge Street in Phonecia when she failed to negotiate a curve and crossed over into the westbound lane and struck a 2019 Dodge Ram 1500 head on. The operator of the pick-up truck was identified as Naftali Klein, age 22, from Brooklyn, New York. Anna Maguire was pronounced deceased at the scene. Naftali Klein suffered minor injuries and was evaluated at the scene by EMS and declined medical treatment.
