Advanced Care Planning….. what does that mean in terms of my health decisions and what are the forms I need to know about?
What is the difference between a Living Will and a MOLST form?
Do I need a DNR and a Health Care Proxy?
Laura Palermo from Hudson Valley Hospice will review and explain these forms and answer any questions you may have about them.
Laura Palermo is also a notary should you want to complete the forms with her help and have them notarized that night!
