State Police investigated a fatal vehicle crash on the Palisades Interstate Parkway

On November 1, 2018, at approximately 6:43 p.m. State Police Haverstraw responded to a single vehicle crash on the Palisades Interstate Parkway south of exit 11 in the town of Clarkstown. Initial investigation revealed that a 2015 Nissan Altima operated by Thomas Kofi, age 68 of Spring Valley was traveling southbound when a deer entered the roadway from the west shoulder. The vehicle struck the deer and veered left off the roadway onto the grass median where it overturned into a wooded area striking several trees. Responding troopers found the passenger, Miriam Kofi, age 50, unconscious in the vehicle. She was removed from the vehicle and CPR was initiated by a trooper. CPR was continued until EMS arrived. Miriam Kofi was transported to Nyak Hospital where she was pronounced deceased. Thomas Kofi was transported to Westchester Medical Center with internal injuries.
