Poughkeepsie, NY – HRHCare Community Health, a nonprofit system of Federally Qualified Health Centers offering a range of medical services for underserved communities throughout the Hudson Valley and Long Island, is pleased to announce that Sherry Wyckoff, MPH, Regional Practice Manager in Dutchess and Columbia Counties, will be presented with the December 2015 Planetree Spirit Award for excellence in patient care. The Regional Practice Manager for HRHCare’s northernmost Health centers in Poughkeepsie, Dover Plains, Pine Plains, Amenia and Hudson, Sherry Wyckoff came to the organization two years ago after working in the occupational health research field for migrant farmworkers at the New York Center for Agricultural Medicine and Health. She has gone above and beyond to make sure that all staffing and patient needs are consistently met at the seven Health Centers in her purview, working tirelessly to see that all Patient Centered Medical Home measures are met. She has also taken a very active role in empowering HRHCare staff through training, developing a presentation on ergonomics and participating in Planetree Refresher courses. Since joining the HRHCare family Sherry has also put her acclaimed culinary skills to use, volunteering to prepare the food served at the Sugar Loaf Sunday Social Fundraising Dinnerbenefitting the HRHCare Alamo Farmworker Center at Goshen. She has volunteered her own time to support the Alamo Hot Meals project and has shared her own vast knowledge of the field with staff working in positions to serve the farmworker population.“Our Planetree approach in treating the whole patient and the tireless work our staff do to connect our patients to the services they need is what sets HRHCare apart,” said Sherry. “The most important thing a patient needs is time – time to listen, time to explain. I feel that our providers, while feeling the time-crunch, still take the time to really connect with our patients.” In her spare time, Sherry loves to garden and cook for friends and family. She also enjoys building stone stairs, retaining walls and garden beds.

“Sherry fosters the Planetree Spirit in all of her Health Center sites,” said Eileen McManus, HRHCare AVP of Operations. “Her compassionate, can-do attitude is clearly evident as she rolls up her sleeves to stand beside her team members to do whatever it takes to get the job done – always with a smile, an encouraging word and some home-baked goodies!”

Planetree is the global leader in advancing patient-centered care solutions, integrating the technology of modern medicine with the healing dimensions of compassion, comfortable surroundings, shared knowledge and freedom to make educated choices. HRHCare has been an affiliate since 1996 and remains the only Federally Qualified Health Center to be a part of this alliance. The HRHCare Planetree Institute was established in 2012 and provides state-of-the-art training to prepare staff in delivering passionate and personalized care. The HRHCare Planetree Spirit Award honors employees who have been nominated by their colleagues for going above and beyond to provide individualized patient care and enhance health care services in their respective facility.
